For Baptism, Confirmation, and First Communion certificate requests, please fill out the form below. All information requested is required so we can locate the correct record and fulfill your request.
Please be aware that we require a copy of your photo ID if you are requesting that a baptism certificate be mailed directly to you. You are welcome to mail a picture of your photo ID (front only) to Christina Deeb at secretary@ourladyofloreto.org. If it is being mailed to a church or school, we do not need a copy of your ID.
ID’s are not required for First Communion or Confirmation notifications.
